Rural Health Medical Program, Incorporated’s mission is to improve the availability of health and wellness services to residents of our service area, by leading the efforts to eliminate the challenges of receiving quality, affordable health care. While creating a seamless system of access to quality healthcare and social services to all residents within our service area. As a HRSA 330 Grant Awardee, we have committed to never turn a patient away regardless of the patient’s ability to pay. As your local network of community health centers, we offer a full range of health-related services, beyond the typical care of other providers such as hospitals or out-patient clinics. This means in addition to providing comprehensive primary health care services. RHMPI provides “supportive services” that include nutrition and wellness education through initiatives like the CYL2 Diabetes Prevention Program, translation services, Chronic Care Management, mail order pharmacy, transportation to and from centers, and outreach activities to increase access to eligible patients.

Rural Health Medical Program, Inc. can now offer mail order pharmacy services through partnership with Curant Health and Capture Rx. This convenient program is a specialty pharmacy that offers our patients the benefit of having medications mailed directly to their home address. Patients pay the same copay that is normally paid to a local retail pharmacy. Visit a RHMPI Health Center to determine your eligibility and enroll in the program. Once enrolled, each patient will be assigned a care coordinator that provides monthly friendly call reminders to make sure they are on track and adhering to medication regiment. This service is critical in supporting patients on their personalized medication pathway.

Transportation is inexpensive compared to the high and rapidly growing cost of healthcare, making it more cost-effective to transport a member to preventative care rather than waiting for a serious health condition to arise. By offering this benefit to the elderly, individuals with disabilities, and other transportation disadvantaged populations, RHMPI strives to:

Health disparities across the Black Belt Region are significantly higher than the state health data. RHMPI provides a variety of health care services to monitor and management our patients with Chronic Diseases. All patients that have been diagnosed with chronic disease are assigned Chronic Care Management Coaches that provide monthly care calls monitoring their overall wellness between regular routine medical visits. Chronic Care Management (CCM) is defined as the non-face-to-face service provided to Medicare beneficiaries who have two or more chronic conditions.
Chronic conditions consist of but are not limited to the following.
- Diabetes
- Asthma
- Arthritis
- Cardiovascular Disease
- HIV/AIDs
- Hypertension
- COPD
- Depression
- Alzheimer’s
- Cancer
In addition to office visits and other face-to- face encounters CCM services include communication with the patient and other treating health professionals for care coordination via telephone, medication management, healthcare assessment, preventative care, dietary/exercise management and having access to health provider/and or staff 24 hours a day. Our Patient Portal is Easy. Convenient. Secure.
We offer 24/7 access to patient resources, files, results, and availability to make payments all on our Patient Portal. Our patients will also have access to chat live with their provider and care management team members via our chat functionality, schedule, update, and/or cancel appointments, request refills for prescriptions, and view account statement and pay bills. The Centers for Medicare & Medicaid (CMS) are mailing new Medicare cards with Medicare Beneficiary Identifier (MBI). The mailing of the new Medicare cards started in August 2020 and is now complete for the Alabama region. For patients who have not received a copy of the new Medicare cards, please sign into your MyMedicare.gov account to check the status or ask one of Rural Health Medical Program, Inc. front desk team members to assist you with obtaining your updated Medicare card information.
